"Well" or "Health Maintenance" Care at

These are rough guidelines for these types of periodic health maintenance (preventative care) exams
  -Some lab, tests, immunizations, checks, may be omitted, because:
    already done, insufficient intervals, uncooperative child, etc.

Daily Nursery Visits (Age 0 to 2-3 days usually):
  Daily minimum, (while your newborn is still in the hospital)
  Vitamin K given, newborn screening blood studies, hearing tests,
  1st Hepatitis B immunization, other studies if necessary.
  AAP Caring for your Infant and Young Child, Birth through 5
    book offered if parents do not already have one and plan long-term care
    with Unifour Pediatrics.

3-4 Day-Old Newborn Office Visit:
  History & physical examination and questions answered.
  Immunizations: usually none. (1st Hepatitis B if refused in hospital.)

  Lab: Usually none.
          - Possible bilirubin transcutaneous check or blood study if needed.
  Handouts provided with health information and anticipatory guidance.

  AAP Caring for your Infant and Young Child, Birth through 5
    book offered if parents do not already have one and plan long-term care
    with Unifour Pediatrics.

2-Week-Old Well Child Check-up

  History & physical examination and questions answered.
  No immunizations. Usually no blood tests.
  Handouts provided with health information and anticipatory guidance.

2-Month-Old Well Child Check-up
  History & physical examination and questions answered.
  Immunizations: DTaP, IPV, HIB, Prevnar, Hepatitis B, and Rotavirus.
  - Usually DTaP, IPV, and HIB combined as Pentacel (one injection).
  - Prevnar and Hepatitis B given separately (2 more injections, so 3 total).
  - Rotavirus immunization is given my mouth (must be started by 12 wks).

  Usually no lab studies.
  Handouts provided with health information and anticipatory guidance.

4-Month-Old Well Child Check-up
  History & physical examination and questions answered.
  Immunizations: DTaP, IPV, HIB, Prevnar, and Rotavirus.
  - Usually DTaP, IPV, and HIB combined as Pentacel (one injection).
  - Prevnar given separately (one more injection, so 2 total).
  - Rotavirus immunization is given my mouth (can't give if started late).

  Usually no lab studies.
  Handouts provided with health information and anticipatory guidance.

6-Month-Old Well Child Check-up
  History & physical examination and questions answered.
  Immunizations: DTaP, IPV, HIB, Prevnar, and Rotavirus.
  - Usually DTaP, IPV, and HIB combined as Pentacel (one injection).
  - Prevnar given separately (one more injection, so 2 total).
  - Rotavirus immunization is given my mouth (can't give if started late).
  - If October-April, 1st Influenza Vaccine, (so possible 3 injections total)
    - (2nd Influenza Vaccine one month later).
  Urinalysis is possible (by urine-bag in diaper).
  Handouts provided with health information and anticipatory guidance.

9-Month-Old Well Child Check-up
  History & physical examination and questions answered.
  Immunizations: Hepatitis B. (one injection, and possibly more if behind)

  - If October-April, & no prior flu vaccine this season, 1st Influenza,

      (so possibly 2 injections total, and more if behind)
    - (2nd Influenza Vaccine one month later, if never had 2 in one season).

  Blood test for anemia (if not already done).
  Urinalysis is possible (by urine-bag in diaper), if unsuccessful previously.

  Dental fluoride varnish for Medicaid and Health Choice patients.
  Handouts provided with health information and anticipatory guidance.

12-Month-Old Well Child Check-up
  History & physical examination and questions answered.
  Immunizations: MMR, Varicella, Prevnar, Hepatitis A,
  - (last HIB
not currently available, but will eventually become available),

  - If October-April, & no prior flu vaccine this season, 1st Influenza,

      (so possibly 5 injections total)
    - (2nd Influenza Vaccine one month later, if never had 2 in one season).
  - MMR is sometimes delayed 1-3 months if 4 immunizations plus blood
     work are already being given, and if the HIB again becomes available.

  Blood test for lead exposure (and for anemia, if not already done).
  Dental fluoride varnish for Medicaid and Health Choice patients.
  Handouts provided with health information and anticipatory guidance.

15-Month-Old Well Child Check-up (Medicaid sometimes this visit)
  History & physical examination and questions answered.
  Immunizations: DTaP,
  - If October-April, & no prior flu vaccine this season, 1st Influenza,
      (so possibly 2 injections total)
    - (2nd Influenza Vaccine one month later, if never had 2 in one season).
  - If missed MMR at 12 months age and not caught up, MMR "catchup",

  Dental fluoride varnish for Medicaid and Health Choice patients.
  Handouts provided with health information and anticipatory guidance.

18-Month-Old Well Child Check-up
 
History & physical examination and questions answered.
  Immunizations: 2nd Hepatitis A (must be 6 months after 1st Hepatitis A).
  - If October-April, & no prior flu vaccine this season, 1st Influenza
      (so possibly 2 injections total)
    - (2nd Influenza Vaccine one month later, if never had 2 in one season).

  Blood test for anemia (unless already had this done).
  Dental fluoride varnish for Medicaid and Health Choice patients.
  Handouts provided with health information and anticipatory guidance.

24-Month-Old Well Child Check-up
  History & physical examination and questions answered.
  Immunizations: usually none.
  - If October-April, & no prior flu vaccine this season, 1st Influenza,
      (so possibly 2 injections total)
    - (2nd Influenza Vaccine one month later).

  Blood test for lead exposure (and for anemia, if not already done).

  Dental fluoride varnish for Medicaid and Health Choice patients.
  Handouts provided with health information and anticipatory guidance.

36-Month-Old (30-47 mo) Well Child Check-up
  --the best times for this visit is actually late-spring or summer.
  History & physical examination and questions answered.

  Immunizations:  usually none.
  - If October-April, & no prior flu vaccine this season, 1st Influenza,
      (so possibly 2 injections total)
    - (2nd Influenza Vaccine one month later).

  Urinalysis if possible.
  Vision screening if possible (depends primarily of cooperation).
  Hearing Screening if possible
(depends primarily of cooperation).
  Dental fluoride varnish for Medicaid/Health Choice if <3 1/2 y.o.
  Handouts provided with health information and anticipatory guidance.

4-6 Years-Old (Pre-school) Well Child Check-up
  --the best times for this visit is actually late-spring or summer.
  History & physical examination and questions answered.
  Immunizations: DTaP, IPV, MMR, and Varicella,  
  - If October-April, & no prior flu vaccine this season, 1st Influenza,
      (so possibly 2 injections total)
    - (2nd Influenza Vaccine one month later).

  Discussion about school readiness, .

  Vision, Hearing, Blood test for anemia, Urinalysis,
  Preschool form filled out for school.
  Handouts provided with health information and anticipatory guidance.

Separate "Sick" and Well" Areas of the Office:
We have always had separate "Sick" and "Well" waiting rooms and exam rooms in the upper level of Unifour Pediatrics. On this upper level, we have set aside areas for well children which are away from children with infectious diseases. We have now gone one step further by having a completely separate level of the office for well newborns, infants, toddlers, and preschoolers. As of July 1, 2008, we have opened the lower level of Unifour Pediatrics for: (a) well children less than five years age and (b) for those who are immuno-compromised (have problems with their ability to fight infectious disease). In this new office, we have independent "well child" parking, waiting rooms, business office, exam rooms and clinical staff. Other sick siblings with the well child will not be permitted in this office (in this case the family will be directed to be seen on the upper level). Those that are "under-immunized" (those who are behind on the STANDARD immunization schedules published by the American Academy of Pediatrics, CDC, NIH, AAFP, etc.) must also be seen on the upper level and NOT in this "Well Clinic", as these also represent a risk to our well and immuno-compromised children seen in the "Well Clinic". We may, in the future extend this "well office" to five year old children as well. Older "well children" will continue to be seen in the upper office, using that office's well waiting rooms and well exam rooms, separate from the sick areas.
